Received this book yesterday but just haven't had the time to come online. Anyways, I have requested a book on October 13 and was marked mailed on October 15th. The book went lost in the system on November 8th. The book finally made it to me yesterday, November 13th, and I never re-ordered that book after it went lost so I know I'm supposed to go into TA and mark it receive when this happens. Only thing I'm confused about is that the PO stamps says November 5th and don't have anything anywhere from the PO stating it was actually mailed on October 15th. Even though it may have been mailed on November 5th, I still have to mark it received from my TA right?

Yes, you can mark it received from the TA and fill in the survey questions. You will be able to put the postmark date in so it will be on record that the book was mailed way after it was marked mailed.

When logging the book in, you can also add additional information at the bottom under comments.  Such as 'this package arrived with USPS counter postage dated 11/5/10, although sender marked it into the system on October 13th' (or 'stamps postmarked 11/5'...or whatever facts fit the situation).

it is important to note in my opinion whether it was counter postage or from what it sounds like in the OP that it was a postal Stamp...now these stamps can be done anywhere along the way and are no real indication of when the book was actually mailed.

Because I take all my books to the PO in person . I actually see them stamp my pkgs. with the dated stamp right in front of me , so I know the dates on my books originate from my mailing. Not so sure when blue box pkgs, get dated. I have even recieved 2 pkgs that didn't have any cancellation stamp at all , no postmark of any kind.  One book just had 2 stamps on it and seemed to have slipped thru the system unnoticed.
