The "disturbing stranger" of this book is so disturbed, he's practically psychotic. He blackmails the heroine into marrying him, humiliates her, slaps her, shakes her and rapes her--but it's all okay, of course, because it's just because he loves her so much. She's pretty disturbed herself, since she falls in love with him. This may qualify as one of the sickest romances I've ever read.
