Do you email author or publisher or is there maybe a site to sign up?  Anybody ever gotten one of these?

I get them from winning contests from authors. You can't just ask for them, they aren't available to just anyone. You also sometimes have to do reviews on them.

I think some people get them because they work for publishers, bookstores, magazines, newspapers etc..,

Right, most people who get them are book reviewers.

I get them every now and then because I'm part of Harlequin's We Hear You panel, basically I review the books after I read them.

The only one I've gotten my hands on was at a used book sale.

Besides getting them from author contests, join all the publisher newsletters  - there are giveaways in most and some do advanced ARC giveaways for relatively simple reviews, comments just before release date.  I've received several over the years.  At Harper Collins, click on First Look and join there, I just got one from there last month.
