The only one that comes to mind is Annie and The Outlaw by Sharon Sala; I don't think he's described as 'angel of death' but he's not mortal and she is terminally ill.

Are your sure it's a full length novel.  I seem to recall reading a short story a few years back where "death" falls in love with a terminally ill woman--serious heart condition and can't expose herself to any kind of exertion.  He takes several days off to be with her and the world goes a little crazy because no one can die until he comes back, even people who have fallen off sky scrapers and broken every bone in their body.

I can't remember right now what anthology that's from, but I'll dig into my boxes and see if I can find it.
