If you post directly to someone's WishList, does that simply mean that person gets first dibs on the book? Does the person spend a credit on the book?

Yes, they will, in effect, jump the WL line. They do have to use a credit, though.

It's the same as any swap, you're just selecting the Requester.  You just have to be sure to click the Post This Book button on their Wish List page, and not the book's Details page, or else you'll just post it to the person first in line.  If the person doesn't accept it, it'll go to the first person in line after their refusal. 

If you want to send it without them having to spend a credit, since it's a WL book, you'll have to generate a Custom DC label.  With posted books you can go to your Bookshelf and on the Details page for the book there's a "Mail it to a friend" link you can use.  I think you need to have their address in advance for these options.
