I know when I have books sent to my mother in Ct she gets an Information for the Receiver like that below (she always reads it to me) which I'm pasting from the Help documents. However I just got an order from a member in Az to send to an alternate address in PA. I didn't get the second page as below when I printed the wrapper, however, I got the usual Info for Receiver. I'm wondering why and should I just type out one myself with notepad?

If you think the sheet that printed might be incorrect, you should report it to PBS so they can fix it. (Contact Us link at bottom of web page).
I don't think it would hurt to print it out yourself, but I wouldn't bother, personally.
I never print or include the second sheet in any package I've ever sent from here, and never had a problem with people knowing who they came from. Usually people ordering for a third person are on top of the issue, and will be asking their person if they go the book.
