Every orthopedic surgeon is familiar with the old s... more »inuses of tubercular joint disease which have persistently resisted all treatments and continued for months and even years. The discharge is a constant drain upon the vitality of the patient. The pus frequently seeks new directions and new outlets until often an old case of Pott's disease or of tubercular hip disease may have three or four discharging and twice as many closed or partly closed sinuses. These cases in a large orthopedic hospital represent alike the neglected patients and the delayed cures or failures of operative, mechanical and other treatments. Especially trying are the abscess cavities and the sinuses which frequently follow excision of the head of the femur. These sinus cases with their daily dressings are a constant source of t rial to the staff surgeons and nurses. January 15, 1908, Dr. Emil G. Beck, of Chicago, reported that while using a bismuth-vaselin paste injection in an old sinus for the purpose of obtaining a clearly outlined skiagram of the track it pursued, he accidentally discovered that the bismuth-vaselin paste was a cure. On February 3, at the Home for Destitute Crippled Children, we began treating 26 of our old tubercular sinus cases with the bismuth-vaselin paste, reserving a number of similar cases for comparison. The formulae for these injections, which we will designate as No. i and No. 2, are as follows: FORMULA NO. I, FOR DIAGNOSIS: H Bismuth subnitrate, i part Vaselin, 2 parts Mix while boiling.
