The strangest thing happened today. I mailed a book on May 12th and it was marked as received. Today, it came back to me unopened with a sticker on it that says "unclaimed." I've never heard of this happening before. Have you?

Take a look at the request details in your Transaction Archive. What date and time was it marked received? If it was marked received just after 3:00 AM on the 25th day after you marked it mailed, then the system marked it received for you because the requestor's account is inactive.
