OK here's a basic rundown of what happened:

1. I ordered a book that was available in the system. It was the only copy but it was not wish listed.

2. The sender accepted the request but did not mail on time and the transaction timed out and was cancelled. The book was placed on my wish list. I was the only wisher.

3. Two days later the sender PM'd me that she had a family crisis and couldn't mail anything which was why she let it time out. She still had the book and would repost it to me. I said OK b/c I really wanted the book.

4. She reposted the book into the system assuming it would automatically go to me as the only wisher. She did NOT post it directly to me.

5. The book never got offered to me and someone else ordered it through the FIFO system, even though it was on my wish list! She accepted their request thinking it was mine and later PM'd me to apologize but she felt she was obligated to send it to them instead because she had already accepted.

6. So why would the book NOT have been offered to me? System glitch, or is there some other possibility I am overlooking? My account is on hold but that shouldn't doesn't affect the wish list does it? I've gotten 3 wish list books this week so I would assume not...

Unless you had the book on autorequest, you WILL get passed up when your account is on hold.  I'd bet that's what happend here.

There is also the slim chance that there was someone else on hold that was technically ahead of you... and when they came off hold they took their rightful slot at the head of the line... very slight chance, but you never know.

Books that the system adds to your wislhist in this way are added as a 48 hour hold (or at least use to be) so with your account on hold that would mean you would have been skipped.

Though I also thought of the ISBN possibility.

Lisa and Kimberly are right - I just checked my wish list and sure enough that book was not on auto-request. Drat. I really wanted to get it before the holidays started and who knows when it will be posted again.

What book is it? You never know, maybe someone else has it but hasn't posted it into the system yet.
