I ordered Dragons of Spring Flame, part of the Dragonlance series. Very pleasant gentleman PM'd me to see if I still wanted with out dust jacket. I said absolutley, he PM'd that it was mailed Saturday. This was in mid May. Of course no book arrived, and the book was never marked mailed. FIFO went to the next one. Still waiting for member since May 24. I'm getting frustrated. When a book is requested from me, I accept it and mail it as quickly as I can. What's up with people?

Hmm.  Most likely the 1st one was mailed and he forgot to mark it mailed.  It's still May now so w/media mail it could take a month to get to you.  I'm still waiting for a swap book that was mailed to me using DC almost a month ago.  The DC# shows as never being scanned.  So who knows where the book is now. 

I would PM the 1st person and ask if he mailed the book because it wasn't marked as being mailed.  You might end up with 2 copies. If  there's several copies in the system-I'd cancel the pending one until I hear from the 1st person.  Give them a deadline-"say I noticed that you didn't mark the book mailed and the request passed on to someone else.  If I don't hear from you by x date, I'm going to reorder the book and I will only mark that one as received if I happen to get 2 books".  Something like that.  If you get 2 books because someone forgot to mark it mailed, you do NOT have to pay a credit to the person who let it time out.  Only to the person who marked it mailed.  But it's common courtesy to try and prevent that.  In this case since he PM'd you w/the question and said he was mailing it then it's highly likely he just forgot to mark it mailed.

It is frustrating when people don't respond quickly to requests.  I do and most do I think.  Some people wait until the last minute to accept because they only mail once a week or need to wait for payday.  I'm good financially and just use the printed postage if I don't have time to go to the PO.  But not everyone can do that.
