If I have a wishlist item on hold (with the red background)... will I still move up in the line if the people ahead of me receive the book or delete the book from their wish list?  If so, will I move up in the line with the item on hold if my entire account is on vacation?  Off vacation?  ie:  I'm #5 on the wishlist for a book but I put it on hold.  Is it possible to still move to 1-4 on hold?  Or will other wishlisters move up ahead of me?  thanks

Your wish list will move with individual Hold or Vacation Hold, you will just not get any offers of the book while on Hold.

People will only move ahead of you get to #1 and stay on hold. Then when the book is posted it'll be offered to the person behind you.  But you will still technicaly be in 1st place.  As soon as you come off hold the next copy posted to FIFO will be offered to you.
