If I accidentally click on "add to wish list" for a book I already have wishlisted (if, for example, I had overlooked it on the list and thought I'd forgotten to wishlist it), does the system know that and ignore it, or does it bump me back to the end of the line?  I'm just trying to figure out whether it penalizes me if I accidentally click it again.

It ignores it. You keep your spot on the wish list.

Don't accidentally stick it on your reminder list though! A book can only be on one of the wish/reminder lists and moving it off the wish list to the reminder list will definitely lose your place.

They have it now so that it says if a book is on your WL, RL, TBR or BIR lists.
