I believe this was a Harlequin, but I'm not sure.

A man and woman have a marriage of convenience.  Something along the lines of the man was engaged to the womans sister and she leaves him at the alter and they end up getting married to quiet the rumors...they are both professionals and secretly love one another. 

The irresponsible sister shows up and dumps a baby on the doorstep.  Taking care of the baby together they eventually come to reveal their true feelings for each other.  Most of the book is them remembering how they met and ended up married together and wishing their marriage was more.

Deadbeat sister eventually shows up to taunt sister about her loveless marriage, but she is wrong!

Just to help you along, I have this one at home and the author is Carole Mortimer.  The heroine is also in love with the hero's brother, but after a year of marriage, their secretly in love with each other.  The plot comes to a head because, Gemma's (the heroine) sister Jemima leaves her baby with the couple and Gemma suspects that it's really her husband.  I'll dig around in my books later and look for the title.

Although, reading your synopsis, I'm not completely sure I'm talking about the same one.  I'll check it out after I get home from work tonight.

OK,

It's:  To Mend A Marriage by Carol Mortimer.

I so don't remember the sister's being twins, but everything else sounds just like the story.

I've requested it and will read it as soon as it comes in!
